He clearly lays blame at the pilot of the helicopter. He explains that the pilot accepted visual responsibility for the CRJ twice. Meaning, the helo pilot claimed to have seen the CRJ twice. This guy thinks he did not actually see the plane he hit, but instead saw a another plane in the distance.
Whether its intentional or not, blame lies a the helicopter pilot.

Excellent video, my irritation comes from the need from this tweet and others to try and frame it immediately as some nefarious action. All legitimate discussion, including from this video have suggested that it was a case of an unfortunate error , in a very difficult location. I haven't heard any of them suggest that it was something deeper than that.
